Can Open Hand help you or a loved one?

How to qualify for Open Hand meal programs.

Senior Clients
Senior clients should contact your local area Office of Aging for client criteria and intake procedures. To view the list of offices in Georgia, click here. Your case management agency will complete the client assessment, then forward a communication form to Open Hand confirming enrollment and notifying us of delivery details and meal type information for each client. To see if you or a loved one qualify for nutrition services, your local area Office of Aging can assist you. Of course, if you simply need additional information or have questions, you may contact Open Hand Client Services at 404-872-6947.

Medicaid Clients | The Elderly and Disabled Waiver Program (EDWP)
If you are elderly and/or functionally impaired or disabled, a Medicaid waiver program such as The Elderly and Disabled Waiver Program (EDWP) may help you return to the community from a nursing home or remain in your own home or the home of a caregiver, as long as possible. To read more about the EDWP Medicaid waiver program, click here. Navigating the process can be a bit confusing, so if at any point you have questions, you might benefit from a live chat with a representative from the program via their Empowerline website. And of course, feel free to contact the Open Hand Client Services department at 404-872-6947.

HIV/AIDS Clients
Open Hand continues to be the primary meal provider in the Atlanta area for our friends and neighbors with an HIV/AIDS diagnosis. Enrollment in our meal service program requires a completed Medical Certification Form signed by a medical provider or Registered Dietitian as well as a Proof of Income or No Income affidavit. To download the Medical Certification Form, click here. If you have any questions, please contact the Open Hand Client Services department at 404-872-6947.

Private Pay Clients & Caregivers
This option was established for those individuals who do not qualify for free Open Hand services, but who can afford to pay a fee for meal service and desire the convenience and nutrition benefits of Open Hand home-delivered meals. Many clients take advantage of this option during a recovery or rehabilitative period. Caregivers are also encouraged to take advantage of this convenient program, as they are prone to put the needs of others before their own and quite often their own health suffers as a result. To get more information or to order meals, simply contact the Open Hand Client Services department at 404-872-6947.
