Using healthy meals and nutrition education as tools to assist them in their efforts to live healthier, independent lives, we work closely with our Registered Dietitians to ensure that our clients are matched with the appropriate program, that they have access to the tools they need to assist them in achieving better health, and that they always receive the highest level of service.
Functioning as a critical link between clients and their families, medical providers, case managers and other resource agencies, our team helps streamline case management and provide an open channel for communication.
